Objective(s): 1 Outline a quality improvement study aimed at increasing conversion of referrals to appointments in a cancer genetics clinic. 1 Assess diagnostic yield of updated multi-gene panel testing in individuals with unexplained adenomatous polyposis and prior uninformative testing 1 Identify patients with colorectal cancer who meet NCCN criteria for genetic counseling and testing 2 Describe the evolution of genetic testing for adenomatous polyposis 4 Identify successful aspects of the QI study and determine if they could be applied to our cancer genetics service. 5 Identify daily challenges experienced by families caring for an individual with AHC. 6 Analyze factors contributing to diagnostic delays in patients with AHC and their impact on affected families. 7 Describe patient experiences with PGT-A informed consent and counseling and explore how these experiences impact decisional regret about testing and embryo transfer decisions. 8 Identify the role of reproductive autonomy and emotional closure as key considerations for patients navigating decisions surrounding non-euploid embryos. 9 Outline how genetic counselors and REI clinicians can support patients with transparent, nonjudgmental counseling throughout the journey from PGT-A to transfer decision-making 11 Hypothesize potential provider-related barriers to appropriate referral to genetic counseling and
