Objective(s): 1 Co-create with the patient a life long plan of care to address the challenges associated with aging with a spinal cord injury or disorder. 2 Evaluate a Dual-Expertise Clinical Workflow: Demonstrate how a formalized partnership between a clinician (PT/ATP) and a technical peer-consultant (IT/Lived Experience) can reduce "Functional Readiness" timelines by at least 20%, leveraging pilot data that pairs clinical assessment with real-world technical troubleshooting. (Keryk, A., et al. 2023) 3 Propose a Quantifiable Regional Integration Framework: Outline the operational components of a regional resource collaborative designed to achieve a measurable reduction in service fragmentation by centralizing cross-system referrals between the VA, Model Systems, and community hospital systems as technical spokes.
