Objective(s): 1 Identify patients with type 2 diabetes mellitus at high risk for incidence and progression of chronic kidney disease and cardiovascular disease events. 2 Describe risk for CKD progression and CVD mortality using the KDIGO heat map. 3 Describe FDA-approved indications for use of non-steroidal MRA, SGLT2 inhibitor, and GLP-1 receptor agonists to reduce risk for CKD and CVD. 4 Identify 3 areas of agreement between nephrology, endocrinology, and cardiology guidelines. 5 Identify 3 gaps in knowledge and practice between nephrology, endocrinology, and cardiology guidelines. 6 Describe 3 ways that University Hospitals can address these gaps to improve knowledge and practice in the care for patients with chronic kidney disease in type 2 diabetes mellitus. 7 Describe 3 ways that University Hospitals can address these gaps to improve knowledge and practice in the care for patients with chronic kidney disease in type 2 diabetes mellitus.
